One-on-One Services Required Documentation

During your registrar appointment, you must provide one document from each relevant category to complete your application and verify eligibility for one-on-one services. All documents must be current and valid. Copies or pictures of documents are acceptable if all pertinent information is legible. Expired or illegible documents can not be accepted.     • Baptismal Certificate (Place of birth must be shown)

    • DD-214, Report of Transfer or Discharge (Place of birth must be shown)

    • Naturalization Certificate

    • U.S. Passport or U.S. Passport Card (Unexpired)

    • Public Assistance Record

    • Voter Notification Card

    • A Letter from the Veterans’ Administration

  • • Selective Service Acknowledgement Letter

    • Registration confirmation from www.sss.gov

    • Form DD-214 “Report of Separation”

    If you are an immigrant, registration with Selective Service is necessary if arrival to the US occurred before your 26th birthday. If arrival occurred after your 26th birthday, registration is not necessary, but you must show proof of age at the time of arrival (such as date stamped passport).

  • Please include 6 months of income for family members, including yourself, living in the same household.

    • Award Letter from Veteran’s Administration

    • Pay Stubs

    • Pension Statement

    • Employer Statement/Contract

    • Quarterly Estimated Tax for Self-Employed Persons

    • Social Security Benefits

    • UI Claim Documents

  • Please bring a verification document for public assistance received by you or a family member, in the last 6 months (for example, determination letter or notice) from the issuing agency

    • Temporary Assistance for Needy Families (TANF)

    • Social Security Insurance (SSI/SSDI)

    • Supplemental Nutrition Assistance Program(SNAP)

    • Refugee Cash Assistance (RCA)

    • Other Income-based Public Assistance
